Sean Archer (John Travolta) FBI-ügynök élete legőrültebb akcióját hajtja végre, amikor orvosi beavatkozással arcot cserél gyűlölt ellenségével, a kómába esett Castor Troy-jal (Nicolas Cage). A megszállott ügynök így próbálja kiszedni a kegyetlen terrorista öccséből, hová rejtették a várost fenyegető bombát. Csakhogy időközben az arc nélküli Troy magához tér és bosszúra éhesen Archer arcával távozik a kórházból.
