You can see how the Joker made his escape from the interrogation room! We get the epic scene between Heath Ledger and Christain Bale, in which Batman beats and throws around the Joker while interrogating him, and at one point we can see the Joker get thrown into glass, shattering it. When the Joker is left alone afterwards with Detective Stevens, you can see the shard of glass behind his head that was from the interrogation, and that he would then use on the detective to escape from his cell.
